2. Call Dr. Johnson's office at 485-2147 to make an appointment with him to schedule your wedding date.

3. Download and print the "Wedding Information Form". This form can also be picked up at the church in Nell Sexton's office. Complete the form.

4. Call Sue Wampler, the Wedding Guild Chairperson, at 864-8391 to schedule an interview. Be sure to bring the completed "Wedding Information Form" with you to this appointment.
